Every week, women walk through the door of my clinic having tried everything — new shampoos, supplements off the internet, treatments recommended by well-meaning friends. And most of them share the same frustration: nothing is working.

Here is why: they have been treating symptoms without addressing the root causes of hair loss.

In honor of Hair Loss Awareness Month, I want to go deeper this week. Real results require understanding what is actually driving your hair loss — and the answer is often more complex, and more treatable, than most people realize.



The Gut-Hair Connection: Why Your Digestive Health Affects Your Hair

The connection between the gut and hair health is one of the most underrecognized factors I encounter in clinical practice. Your gut microbiome — the ecosystem of bacteria, fungi, and other microorganisms living in your digestive tract — plays a direct role in nutrient absorption, hormone regulation, and systemic inflammation.

When the gut is compromised (a condition often called "leaky gut" or intestinal dysbiosis), the body cannot effectively absorb the nutrients that hair follicles depend on for growth. Even if you are eating a balanced diet, poor gut health can leave your hair starved at the follicular level.


The gut also metabolizes and eliminates excess hormones. When this process is disrupted, hormonal imbalances can develop — and hormones, as we will discuss below, are among the most powerful drivers of hair loss in women.

In my holistic approach to trichology, we always look at the full picture: what you eat, how your body absorbs it, and how it affects hair growth from the inside out.


How Chronic Inflammation Triggers Hair Shedding

Inflammation is the body's natural defense mechanism — but when it becomes chronic, it turns destructive. Chronic inflammation can attack hair follicles directly, disrupt the growth cycle, and create a scalp environment that is hostile to healthy hair production.


Sources of chronic inflammation include:

  • Poor diet high in processed foods, refined sugar, and seed oils

  • Chronic stress and elevated cortisol

  • Autoimmune conditions

  • Undiagnosed food sensitivities (gluten, dairy, and soy are common culprits)

  • Environmental toxins and chemical exposure — including harsh hair care products


One of the most consistent findings I see in clients with persistent hair shedding is an underlying inflammatory condition that has gone unaddressed. Treating the inflammation is often the turning point in restoring healthy hair growth.



Hormones and Hair Loss: The Three Primary Culprits

Hormonal imbalances are among the most common causes of hair loss in women, particularly during periods of hormonal transition.


Menopause

During perimenopause and menopause, estrogen and progesterone levels decline significantly. These hormones are protective for hair — they keep follicles in the growth phase longer and reduce the effects of DHT (the hormone responsible for follicle miniaturization). As estrogen drops, hair follicles become more susceptible to DHT activity, resulting in diffuse thinning and density loss. This is often the first point at which women over 40 notice their hair changing dramatically.


Polycystic Ovary Syndrome (PCOS)

PCOS is one of the most common hormonal disorders in women of reproductive age, affecting up to 10% of women worldwide. It is characterized by elevated androgens (male hormones), including testosterone and DHT, which directly contribute to hair thinning and follicle miniaturization — the same process seen in androgenetic alopecia. Women with PCOS may also experience irregular periods, weight gain, and acne alongside their hair loss.


Thyroid Dysfunction

Both hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid) can cause diffuse hair loss. The thyroid regulates metabolism and plays a central role in hair follicle cycling. When thyroid hormones are dysregulated, follicles can prematurely shift out of the growth phase, leading to widespread shedding. Thyroid-related hair loss is frequently missed because standard blood panels do not always include the full thyroid panel needed to catch subclinical dysfunction.

The Top 5 Nutritional Deficiencies That Cause Hair Loss

What you eat — and what your body absorbs — has a profound effect on hair growth. These five nutritional deficiencies are among the most common contributors to hair loss in women:


1. Iron Deficiency

Iron is essential for producing hemoglobin, which carries oxygen to cells — including hair follicle cells. Low ferritin (stored iron) is one of the most frequently overlooked causes of hair shedding in women, particularly those with heavy periods. Hair loss caused by iron deficiency often presents as diffuse shedding rather than patterned loss.


2. Zinc

Zinc supports protein synthesis and cell division — both critical to hair growth. Zinc deficiency can result in dry, brittle hair, scalp issues, and increased shedding. Stress, excessive sweating, and vegetarian diets are common risk factors for zinc depletion.


3. Vitamin B12

B12 is essential for red blood cell production and nerve health. A deficiency reduces oxygen delivery to hair follicles and disrupts the hair growth cycle. Vegans, vegetarians, and individuals over 50 are at particular risk, as B12 is found primarily in animal products and absorption decreases with age.


4. Vitamin D

Vitamin D receptors are found in hair follicles, and emerging research consistently links low vitamin D levels to alopecia areata and other forms of hair loss. Despite living in sunny South Florida, many of my clients test low in vitamin D due to indoor lifestyles and limited dietary intake. Optimal hair health requires optimal blood levels — not just within normal range.


5. Protein

Hair is made of keratin, a structural protein. When dietary protein is insufficient, the body prioritizes vital organs over hair production, triggering increased shedding. Crash dieting, restrictive eating, and certain medical conditions that impair protein absorption are common culprits.



Scalp Health Is the Foundation of Hair Growth

All hair growth originates at the scalp. If the scalp environment is compromised — whether by inflammation, buildup, poor circulation, or microbial imbalance — hair production suffers regardless of how many supplements you take.


A healthy scalp requires:

  • Proper sebum balance (not too oily, not too dry)

  • Clean, unclogged follicles

  • Adequate blood circulation to deliver nutrients

  • A balanced scalp microbiome

  • Freedom from chronic inflammation
